Perth-based Juniper’s Annesley Transition Care facility expected to fill within 10 days

1 min read

Juniper welcomed the first residents to its 16-bed Juniper Annesley Transition Care Program (TCP) facility on 1 October, and expects the home to be filled today.

Annesley TCP, built by Cooper and Oxley, is a refurbished wing of the existing Juniper Annesley Residential Aged Care Home in Bentley, 8km south of Perth's CBD.

Juniper operates TCPs in Juniper Hayloft in Martin, southwestern Perth, a small-house model at Juniper TCP Karrinyup, which opened in July, as well as TCP support provided in people’s own homes.

TCPs are an initiative of the Western Australian Government and provide short-term care for up to 12 weeks for those who need additional support at the end of a hospital stay following injury or illness. Residents receive around-the-clock care and support from physios and occupational therapists, with the facilities designed to help ease pressure on overcrowded WA hospitals.

Juniper CEO Russell Bricknell said the TCP model helps people regain their skills and confidence so they can carry out daily activities at home.

“Our program adopts a reablement approach, focusing on person-centred care to help people regain  independence and improve functional abilities after illness or injury,” Russell said.
