‘Small home’ for Transition Care opens at Juniper’s Karrinyup site

Western Australia's largest Not For Profit aged care provider Juniper has expanded its Transition Care Program (TCP) to Karrinyup, Perth, with the construction of two homes based on the 'small house' model of care.

The first home opened on Tuesday, with the second opening shortly, and are in part aimed at easing pressure on public hospital beds.

Juniper's Karrinyup location is multiservice with retirement living, residential aged care and now TCP.

It will be the first time the small household model of care has been employed at the Karrinyup site, and will provide a home-like environment for residents. Residents will be able to access around-the-clock care from an on-site registered nurse and support from Juniper’s team of allied health professionals, including physiotherapists and occupational therapists.

“Unlike some of the traditional Transition Care offerings, a small-house model can benefit many of our customers who aren’t quite ready to go home after their hospital stay but would benefit from being able to practice everyday tasks in a home-like environment whilst receiving expert clinical and personal care,’ said Juniper CEO Russell Bricknell.

Each home will have seven bedrooms with ensuites, and a shared kitchen and living spaces. In the communal areas, residents will be able to engage in everyday activities. The TCP small house model also encourages social interaction and a sense of community, which can help to maintain mental and emotional health.

The TCP provides short-term care for up to 12 weeks after a period of hospitalisation, either at a residential home, the person's own home, or both, with a focus on improving health and mobility to return to independent living.

The TCP expansion is part of broader efforts to ease pressure on public hospitals in Western Australia.
