End-of-life befalls every human and there are ways to ensure it goes as smoothly as possible when a loved one enters residential aged care.

Jo Boylan, Chief Executive Officer at Adelaide-based Clayton Church Homes, said advanced care planning helps to understand a person’s choices and makes a significant difference in the quality of end-of-life care. Failing to plan escalates the pressures and emotions dramatically.

Jo said too often health literacy and decision making comes in a crisis, as advance care planning was delayed or avoided even when entering aged care services.
